General Information
We make every effort to provide you the highest quality care possible. To do this, you share in that responsibility. By following the guidelines below, you will help yourself and members of your family utilize our services as efficiently as possible.
New Patients
Please print and fill out the new patient forms to expedite your registration with us. This will greatly shorten your wait time.
Scheduling
Please call 541-476-7775 for appointments. We will try to schedule to your convenience. Urgent appointments will be scheduled as soon as possible-usually on the same or following day. Urgent appointments may not allow you a choice of provider. Routine appointments will usually be able to have you see the provider of your choice. We strive to see each patient at their appointed time, however, emergencies or unscheduled demands on the provider may sometimes lead to delays.
Telephone Inquiries
Our office staff has been instructed to contact the provider before providing any medical recommendations. In most instances one of the providers will be available in the office and most questions can be handled immediately. Because of the providers responsibilities you may not be able to speak to the doctor immediately when you call, however, every medical question will be relayed to the doctor in a timely manner.
Medication refills
Refills on routine medications should be made during regular office hours, Monday through Friday. Some medications can be refilled with a phone call, other times it may be necessary to be seen prior to medication refill. Although symptoms can be similar, the disease pattern may be different & a new medication may be more appropriate. If you need refill of current medication it is helpful to have the prescription number & pharmacy name. You may contact your pharmacy even though you don’t have refills and they will contact our office.
After Hours
The providers share "call" on a rotating basis. If there is an after hours or weekend problem you should call our main number at 541-476-7775 and our answering service will contact you with the provider "on-call". If you have just had surgery, please inform the operator and in most cases you will be connected with your surgeon.
Managed Care
If you belong to a "Managed-Care" insurance you will be required to have a referral from your primary care physician before you can be seen.
Billing
When provided with the proper insurance information, we will be happy to bill for your visit. We are contracted with many insurances, however, it may be best to check with us prior to your visit.
Co-payments are due at each visit. Some procedures may not be covered by your copayment and may become patient responsibility. If you have questions regarding billing our staff will be happy to assist you. Our providers may not be as up to date as the staff with regards to what insurance may or may not cover.
Payments
All charges incurred are payable at the time of service. Regardless of your insurance coverage, our office requires you to settle your account. We do offer several forms of payment.
Payment in full at time of service is eligible for a discount-please inquire with the staff if you are a private pay patient.
Deductibles, co-payments or any portion insurance may not cover.
Payment of balance in full upon receipt of your billing statement.
Special circumstances may allow you to be put on a payment plan. This should be discussed with our billing manager prior to your visit.
